Center for Disease Control Foundation/ Adult Mental Health Division, Honolulu HI  2022 to Current

Contracted to work with the Adult Mental Health Division with the Department of Health in the State of Hawaii, to assist in assessing and connecting individual’s suffering from severe mental illness to supportive services and programs. Submitted authorizations for services rendered or to be rendered by crisis stabilization and case management programs.  Reviewed and processed eligibility documentation submitted by the Hospitals and Correctional Facilities to authorize and link services for qualified individuals.  CDC contract ended December 2022 and was hired by the Adult Mental Health Division, State of Hawaii.

 

CLINICAL CASE MANAGER

Mental Health Kokua, Honolulu HI 2019 – 2022

Assisted clients with Severe Mental Illness to manage mental health symptoms, living needs, and in achieving personal goals to improve their quality of living, through the provision of Case Management support and connection to psychiatric therapeutic, and other community-based services.  Managed client cases by applying research on treament modules for mental illness in utilizing a person-centered approach, to help emplower clients in gaining greater independence and stability.

Conducted Individual and Group counseling services for men suffering from addiction and assisting in conducting interviews and treatment planning for clients.
